The supplier is an individual and provides the payer this written statement to the effect that the supply is made in the course or furtherance of an activity done as a private recreational pursuit or hobby.

I understand that participation in the assigned sports event or activity could include actions or tasks which may be hazardous to me, the service provider named on this form. I voluntarily accept and assume any risk of harm or injury which might occur to me due to my participation in the event or activity, or occasioned by my provision of the service.

I hereby indemnify and hold harmless PE Sport Group Pty Ltd trading as PE Sport Specialists™, and SportsGuard®, for any and all liability for loss and damage (including property damage and consequential loss), or costs which might arise as a result of my participation in the event or activity or my provision of the above named service (whether such loss and damage is caused to me or to any third party).

I further acknowledge that I am responsible for my legal liability in respect of personal injury and/or property damage suffered by a third party which arises as a result of my negligent acts or omissions or my breach of any contract (actual or implied) for the provision of the assigned service.
